The End of Life Care Practice Development Team, based at Cynthia Spencer Hospice, consists of experienced Palliative care specialist nurses who work in close collaboration with the Specialist Palliative Care Service.
The team’s key function is to facilitate the delivery of high quality end of life care, through education, irrespective of the patient’s diagnosis or place of care. The team promotes and supports the implementation of end of life care tools such as the Liverpool Care Pathway and the Gold Standards Framework as identified in the End of Life Care Strategy (DH, 2008).
Although the team do not work directly with patients they aim to positively influence the quality of care by providing education, advice and support to staff in Palliative Care units, Care Homes and the community regarding end of life care issues.
The team also hosts and delivers End of Life Care education on a variety of current topics.
